Essentially, the main game is this:
You have to put down the same number of cards as the first player until everyone passes. So if the first player put down 3 spinners, everyone has to put down three of the same cards and they have to be spinners (rank 7) or lower (lower numbered cards are better). Then there will be a point where no one else can go (or they can go but they're being tactical and don't want to. In that case, they just say 'pass'). So the last person to put cards down gets to start the next go. If they put down one card, then everyone has to do the same. If they put down 5, then everyone has to (try to) do the same. You keep going like this until you get rid of all of your cards. The first person to do that becomes the Greater Dalmuti for the next game. If you're the second person to go out, you become the Lesser Dalmuti in the next game. If you're the second to last person, you become the Lesser Dreg, and if you're the loser, you're the Greater Dreg.
That's the main game. All of the rest of the rules fit into place around that, and you can just keep playing for ages. The artwork is really great. If you do need to google any rules for clarification, it's worth noting that in the old game, the dregs were called the greater and lesser peon, the jokers are called jesters and some of the other characters have different names too. The shakedown was called 'taxation' (which I think was better) and the monster riot was called 'revolution' (which I also liked better).
